This assessment forms part of our determination as to whether you are deemed to be under direct supervision, direction or control on your current or upcoming assignment with We Contract. We will also confirm this assessment with your agency who may be able to provide further information on your assignment from the end client. Should your circumstances change at any time you must inform us immediately. Your answers to this questionnaire will be held on record and used as supporting documentation for should we determine you to be eligible for the reimbursement of tax free expenses.
